
U.S. hospitals are operating under mounting economic pressure as inflation and rising costs continue to outpace reimbursements — creating what many call a “perfect storm” for healthcare providers. According to a report by American Hospital Association (AHA), hospitals and health systems are seeing steep increases in expenses for labor, drugs, supplies and other core costs — while funding from public payers fails to keep pace.
In 2024 alone, total hospital expenses grew 5.1%, significantly above the broader economy’s inflation rate of 2.9%. Labor remains the largest cost driver; ongoing workforce shortages have forced hospitals to raise wages to recruit and retain staff, contributing heavily to the increased cost burden.
At the same time, reimbursements from public programs continue to lag. In 2023, hospitals reportedly received just about 83–82 cents for every dollar spent caring for patients under Medicare, contributing to a cumulative underpayment estimated at around $130 billion that year.
This gap grows wider when inflation is taken into account: from 2022‑2024 general inflation rose faster than reimbursement rates for inpatient care.
Non‑labor costs — including medical supplies, drugs, and other operational expenses — are also creeping up. A recent industry‑wide review found supply costs represented roughly 13% of total hospital expenses in 2024, with steep year-over-year increases among a set of 20 health systems, Becker’s Hospital Review reported.
Taken together, the trend is putting pressure on hospitals’ ability to maintain services, invest in infrastructure, and meet community needs. According to the AHA, many systems face growing financial strain that may threaten access to care, especially in underserved or rural areas.
