UPMC Identifies Community Health Needs

UPMC's 2025 Community Health Needs Assessments (CHNA) have identified four key health priorities across the regions it serves:

  1. Chronic Disease Management: Addressing the prevalence and management of chronic conditions.
  2. Behavioral Health: Focusing on mental health services and support.
  3. Access to Care and Navigating Resources: Improving access to healthcare services and assisting individuals in navigating available resources.
  4. Prevention and Community-Wide Healthy Living: Promoting preventive measures and fostering healthy living within communities.

These priorities were established through a comprehensive assessment process involving public health data analysis, examination of socioeconomic factors, and feedback from over 3,600 community stakeholders, including leaders and organizations representing medically underserved and low-income populations. 

"Engaging with community leaders, public health experts and stakeholders throughout the needs assessment process helps us positively impact the health of the people in the communities we serve," said Donald M. Yealy, M.D., chief medical officer and senior vice president of the Health Services Division at UPMC. "The insights gained from this process guide how we fulfill our commitment to improving community health.”

UPMC's approach includes developing hospital-specific implementation plans to address these identified needs, ensuring that efforts are meaningful, impactful, and equitable
