Hackensack Meridian Health Expands Successful Program to Two New Sites

Hackensack Meridian Health announced that its Hospital From Home program has been extended to Ocean University Medical Center (Brick, NJ) and Southern Ocean Medical Center (Stafford Township, NJ), bringing the total participating sites to five hospitals. The program previously launched at Hackensack UMC, JFK UMC, and Jersey Shore UMC

Since its debut in April 2024 (following a pilot in 2022), the program has provided hospital-level care at home to more than 1,000 patients, demonstrating both growth and patient acceptance.

The service includes a hybrid care model: 24/7 virtual monitoring via a physician-led command center (partnered with Medically Home/DispatchHealth) along with scheduled in‑person nursing visits. Care delivered at home encompasses IV antibiotics, infusions, medication management, labs, various therapies (physical, occupational, speech, respiratory), imaging, meals, and nutritional counseling.

Supported by clinical research, Hospital From Home delivers better patient outcomes, higher patient and clinician satisfaction, cost savings, reduced mortality, and lower readmission rates. Admitting physicians evaluate each patient for eligibility, and the service is widely covered by major payors including Medicare, Cigna, Horizon BCBS of NJ, and UnitedHealthcare
